For your first visit: Please arrive 10-15 minutes early to register with the front office prior to your appointment time. Bring all insurance information, driver's license, and a physician referral if required by your insurance company. Your physician's office may fax a prescription directly to our office. (Fax # 501-224-5460) Print and fill out patient forms to the left and bring with you to your appointment. Bring a list of all current medications and any test results which may be applicable ( MRI reports, nerve conduction reports, x-rays reports) Expect your first visit to last 1 -1.5 hours. Wear comfortable clothing and shoes. Be prepared to pay your copay (if applicable). * All evaluations will be performed by a licensed physical therapist in a comfortable and private setting.

Insurance information Advanced Physical Therapy of Little Rock will verify insurance benefits and file all insurances, both in network and out of network as a courtesy to our patients. If your plan is out of network, we will help you weigh your options regarding physical therapy treatment. Please contact our front office staff to discuss any questions you may have regarding the specifics of your insurance plan.
