NEW PATIENTS

 

Please arrive 15-20 minutes early to register with the front office prior to your appointment time. Bring your driver's license, all insurance information and a referral from your physician (if you were given one). Your physician's office may fax a prescription directly to the appropriate office:

Bring a list of all medications you are currently taking and any test results which may be applicable (MRI reports, nerve conduction reports, X-rays, etc.).

Your visit will last around an hour. Orthopedic patients should wear comfortable shoes and clothing. All evaluations are performed by a licensed physical therapist in a comfortable and private setting.

Please keep in mind that your appointment time is not an estimate. It is very important that you arrive on time for your appointment and be respectful of other patients' appointment times. If you are late for your appointment then your treatment may have to be shortened and you may not receive the full benefit of you treatment for that day. Please call ahead to let us know if you think you may be late for an appointment.

Most orthopedic patients are seen three times a week for 4-6 weeks and pelvic floor patients are typically seen twice weekly for 4-6 weeks. Every treatment is important and you will progress faster if you commit to attend all your appointments. If you must miss a scheduled appointment please call us as soon as you know this and we will do our best to reschedule your appointment.
